Dermal Fillers: Why Are They Criticized?
Fillers have a reputation problem — but the issue is not the product, it is who injects it and how.

Hyaluronic acid fillers have been one of the most studied and refined aesthetic products of the last two decades. Used well, they restore lost volume, sculpt the cheekbones, lift the midface and soften deep folds, all in a way that respects the patient's natural features.
So why do they get such a bad reputation?
The problem is not the filler
The "filler face" so often mocked online is rarely the product's fault. It is the result of three avoidable mistakes:
- Over-filling — chasing volume instead of structure, year after year, until the face loses its proportions.
- Wrong product in the wrong plane — using a thick, structural filler where a soft, flowing one belongs, or vice versa.
- Inexperienced injectors — too often, fillers are placed by practitioners without medical training in facial anatomy, vascular danger zones or aging dynamics.
What good filler work looks like
When Dr. Palermo plans a filler treatment, the conversation begins with the patient's face as a whole — not with the wrinkle the patient came in to point at. The goal is to identify where volume has been lost (most often the deep midface, temples and jawline) and to restore exactly that, in tiny, controlled amounts.
A good outcome is invisible. Friends notice the patient looks rested, but cannot say why. Nothing looks "done." Movement is preserved.
Safety first
Hyaluronic acid fillers are reversible — a key safety advantage. If something is uneven or undesired, hyaluronidase dissolves the product within hours. This reversibility is why HA fillers remain Dr. Palermo's first choice for nearly all volumization work.
The honest verdict
Fillers are not the enemy. Aggressive marketing, untrained injectors and a culture of "more is more" are. In skilled hands, in conservative doses, with a long-term plan that respects how the face matures, fillers remain one of the safest and most rewarding tools in aesthetic medicine.
